Toast Payroll: Form 8655 Reporting Agent Authorization

Form 8655 Summary

Completing and submitting Form 8655 will set Toast Payroll as the reporting agent for your business and allow us to file quarterly payroll taxes on your behalf. You can digitally complete and submit this form on the Toast Payroll website. See pages 2 and 3 of the form on the IRS website to learn more about the purpose of this form, including who is allowed to fill out and sign it.

Form 8655 Task in the Tax Center

HR+ users can visit the Tax Center to find all tasks that must be completed to help enable a successful quarterly tax filing. By checking the Tax Center each quarter and completing every task here, you can plan for upcoming payroll tax filings and ensure Toast Payroll has the items necessary for your payroll tax filings.

 

  1. In Toast Payroll, navigate to Taxes.
  2. If you see Form 8655 as a task in the Tax Center, select Submit on the task. Be sure to do this for each FEIN in Toast Payroll.

  3. The Taxpayer portion of the form will be pre-filled with the information you've already submitted to Toast Payroll. You'll only need to enter a Contact person and Daytime telephone number; the Fax number field is optional.
    1. If any of the pre-filled information is incorrect, contact Toast Customer Care via the blue button in the lower-right corner.
  4. Scroll down to read the Authorization Agreement and fill out the Title and Date fields.
  5. Confirm your authorization by electronically signing the document in the large box above the legal attestation.
  6. Finally, to complete the form, select the legal attestation checkbox and press Submit (upper-right corner).
  7. If you'd like to view this completed form at a later time, navigate to Reports > Tax Filing and select Download in the green banner.

Form 8655 Alert in Payroll Processing

If your business has not already filled out this form for each applicable FEIN, we will alert HR+ users to do so in two ways, the first being the Tax Center task described above. Additionally, after selecting Payroll to open the Pay Cycle Dashboard, any FEIN that does not have the form filled out and submitted to Toast Payroll will see a red banner alerting you of this requirement.

 

Select Submit on the banner and use the instructions in the section directly above to guide you through completing this form. We highly recommend doing this as soon as possible.

  • If you have more than one FEIN without a submitted Form 8655, you'll first see a table with each FEIN and its form completion status. Select Start for each FEIN without the Submitted status and fill out the form completely.
  • If you only have one FEIN, you will be taken directly to the form.

Download Form 8655

If your business has already submitted Form 8655 to Toast Payroll for each FEIN in Toast Payroll, you will not see the red banner described above. Instead, by navigating to Reports > Tax Filing, HR+ users will see a green banner where you can download the form(s) for your records. Select Download to receive your signed and completed Form 8655.

  • The form may have a previous year on it, but that is acceptable. You do not need to fill out a new Form 8655 for each FEIN every year as long as we have one on file.

Terminate or Revoke Authorization

Once Form 8655 is signed, any authority granted is effective beginning with the period indicated on lines 15, 16, 18a, 18b, and/or 18c and continues indefinitely unless terminated or revoked by the taxpayer or reporting agent. No authorization or authority is granted for periods prior to the period(s) indicated on Form 8655.

 

To terminate or revoke your authorization, you'll need to:

  • Fill out a new Form 8655 with your new payroll provider
  • Notify Toast that you won't be continuing with our services so that we no longer have power of attorney
  • Once those steps are complete, download your Form 8655 in Toast Payroll, write "REVOKE" across the top, and mail it to the IRS at:
